All student-athletes are encouraged to attend, including those students in the Class of 2016.
Please arrive to the C-M Library on May 19 according to your class year or sport as follows:

Cost - $20.00 per student, payable by cash or check written out to Chaminade-Madonna College Preparatory.
Physicals will be conducted by University of Miami UHealth Sports Medicine medical staff.
A completed physical is required by the FHSAA for all athletic activities including tryouts, workouts, conditioning, practice, and competition. Please be advised that any student-athlete without a completed physical will not be permitted to participate in athletic activity at Chaminade-Madonna.
PLEASE NOTE:
*ALL athletic physicals are due to Mr. Emery (Athletic Trainer) no later than July 1, 2012.
*Athletic Physicals are valid for the entire 2012-2013 academic calender year.
*Please CLICK HERE to print the 2012-2013 physical packet and complete all paperwork prior to the physical date.
*Please remember to bring your son/daughter's health insurance card to the physical. |
